A web-based application designed to facilitate coordination between Community Health Centers and Mental Hospitals in the care of patients with mental disorders. This application belongs to a mental hospital in Aceh and is not available to the public.

Background

Mental hospitals and community health centers (Puskesmas) face challenges in monitoring patients with mental disorders. Currently, the patient referral process from Puskesmas to mental hospitals is often inefficient:

  • Patient data is not integrated.

  • Coordination between health facilities is slow.

  • Mental hospitals struggle to sort out which patients require immediate treatment.

The client desired an integrated digital system that could:

Connect community health centers (Puskesmas) with mental hospitals.

Accelerate the decision-making process for patient care.

Present patient data in real time and easily understood.

I discussed with the senior management of the Mental Hospital and the medical staff at the Community Health Center to understand their needs.

User Persona

Focus on patient data input and evaluation reports of patients who have been discharged, only need concise data to decide on treatment.

Community Health Center Nurse

The community health center doesn't know when a patient has been discharged. It needs detailed information to monitor and evaluate discharged patients.

Pain Points

  • Hospitals cannot track patients' condition after discharge. Even if data is received, it is not in real time.

  • Community health centers do not know when patients are discharged.

  • Community health centers have difficulty obtaining information about patients' medical histories.

Activity Diagram

The purpose of an activity diagram is to understand the dynamic behavior of a system by showing the flow of activities carried out by actors and how the system responds to these activities.

Use Case Diagram

In this system, two actors interact with the application: mental hospital staff and community health center staff. The actor-system interactions include login, patient data management, symptom recording, patient discharge, and post-discharge evaluation.

Login Page

The Login page is the main entry point for Mental Hospital Staff to access the system. This page ensures that only authorized users can access the application.

Dashboard

This Dashboard page is designed to allow users to get an overview of the patient's condition quickly, clearly, and interactively.

Patient Data

The Patient Data Page is used by Mental Hospital staff to manage detailed patient information, ranging from identity data, treatment status, patient discharges, to medical history.

Notification Page

The Notifications page displays the latest information regarding patient evaluation reports from the Community Health Center to the Mental Hospital staff. Its purpose is to enable the Mental Hospital to promptly follow up if a patient requires further treatment.

Prototype testing with doctors and administrative staff directly.

Feedback

  • The hospital wants referral notifications to be more prominent.

  • The community health center wants a more concise graph with time filtering.

  • There are several additional forms, such as supporting documents when adding new patient data.

Improvements

  • Adding time filters (1 week, 1 month, 1 year)

  • Enhancing the notification colors.

  • Added document upload section when adding new patient data.
